• Certified copy of the death certificate issued by the Department of Home Affairs

• Certified copy of the deceased’s ID

• Certified copy of the beneficiary’s ID

• Fully completed BI form • Police or accident report (MVA/PVA) if the death was due to unnatural causes

• Proof of bank account – recent bank statement or stamped confirmation letter (not older than 3 months)
